Here’s what I found misses when people under-perform, making business fail:

Lack of clarity, purpose, alignment to focus actions to prevent waste of time/energy/money;

Poor productivity practices – distractions, procrastination, messy email and communication practices, poor personal energy management which leads to poor quality of work, loss of clients, business failure;

Lack of resilience practices – stress regulation to prevent burnout and withstand setbacks, fatigue and overwhelm, loss of motivation, innovation, initiative which leads to poor decisions, stagnation, ineffective leadership and toxic culture.
